Benchmark Health, Inc. and our subsidiaries and affiliates (“Benchmark Health”, “us”, “our”, or “we”) are committed to protecting the privacy of individuals, health care providers, patients, customers, and website visitors (“you” or “your”). We created this Privacy Policy (the “Policy”) to inform you of our practices regarding the collection, use, and disclosure of your information when you visit our website or use any of our online services (collectively, the “Online Services”). Please read this Policy carefully to understand our policies and practices regarding your information and how we use and protect it.Please note that the health information that we collect through the Online Services may be subject to the federal Health Insurance Portability and Accountability Act (“HIPAA”). We may collect, process, and handle protected health information (“PHI”) on behalf of our health care provider customers under the business associate agreements (BAAs) that are in place with those customers who are subject to HIPAA. Under those circumstances, such information will be subject to those health care provider’s notice of privacy practices. Please see your health care provider’s notice for more information on how we handle PHI and your rights in regard to your PHI.This Policy applies to the Online Services that we own or operate and that contain a link to this Policy. This Policy applies to all personal information obtained as a result of individuals using the Online Services and who contact us via phone or email, as well as personal information obtained from third parties. While certain information we collect and use will be controlled by our health care provider customers pursuant to HIPAA and their respective notice of privacy practices, this Policy provides an overview of all of the information we may collect from or about you to ensure you have a full understanding of the scope of our information practices.

WHAT TYPES OF INFORMATION WE COLLECT ABOUT YOU

‍TEXT MESSAGES
‍Some services may allow you to enroll in our text message program for service-related and/or promotional messages. If you affirmatively consent to receive text messages from us and enroll in such a program, you will receive updates via text unless you opt-out of the text message program. These messages may be unencrypted. Message frequency may vary based on the services. Message frequency may vary based on individual care needs and engagement. If at any time you would like to stop receiving text message notifications, please text “STOP” in reply to our message, we will send you a reply message to confirm that you have been unsubscribed and you will no longer receive messages from us. If you want to join again, just sign up as you did the first time and we will start sending messages to you again. If at any time you forget what keywords are supported, just text “HELP” back to us and we will respond with instructions on how to use our service as well as how to unsubscribe. Mobile information will not be shared with third parties/affiliates for marketing/promotional purposes.

‍CHILDREN UNDER THE AGE OF 13
‍Our Online Services are not intended for children under 13 years of age. No one under age 13 may provide any information to us through use of the Online Services. We do not knowingly collect personal information from children under 13 unless we have verification of parental or guardian consent. If you are under 13, do not use or provide any information to us through the Online Services.
